    Important information related to NCAA Eligibility Below

    2021-2022 Required clearance paperwork

    Please contact the Athletic Trainer Mr. Ochoa at enrioque.ochoa@aps.edu and your coach to receive information on how to turn in the 2021-2022 NMAA physical packet.

    It is required for ALL student athletes to be cleared by the Athletic Trainer for any out-of-season or in-season participation.  Please turn in your physical packet to the Athletic Trainer only.  

    The 20-21 NMAA physical packet posted below is the most current and should be used for the 21-22 school year.  This is the only packet you will turn in to the Athletic Trainer.  Any outdated forms will not be accepted.  

     

    Physical Packet turn in dates/Location

    Take one of the vehicle entrances off of Rainbow and use the service road to access the East side of the school.  Park on East side of the school near the sport fields.  The outdoor concession stand is directly under the Volcano Vista Hawks that is painted on the building, directly West of the Football field.  Please wear your mask and stay socially distanced while in line.  

    Tuesday, June 15. 915am-11am

     

    NFHS concussion certificates, Activity code of conduct, APS compete with class form, and the APS consent to Participate form must be dated on or after April 1, 2021 to be accepted.   Only these forms will be turned in electronically prior to competition early in the school year. Information for turning in these forms will be posted when school begins in August.  

    Additional Tryout Information

    1.  Must have 2021-2022 NMAA Physical Packet filled out and turned in to Athletic Trainer (please make copy for your own records)

    2.  Must be registered or pre-registered at VVHS and all enrollment documentation must be complete and accurate.

    3.  If you make the team, you will need additional forms that will be TBA.

    All 11th grade student/athletes need to register with the NCAA and be aware of the NCAA requirements.  A free course is available to review all information at the NFHS Learning Center . Also, check out an Interactive Presentation by the NCAA
